US Clinical Rotations in Texas: Where IMGs Can Train

Texas offers something most states on our roster do not: a true inpatient sub-internship. Between an Internal Medicine placement in McAllen and three outpatient rotations in Houston and Richmond, Texas covers a wider range of clinical settings than almost any other state we work with.

An Actual Sub-Internship in McAllen

The Internal Medicine Sub-Internship at South Texas Health System (DHR Health & Cornerstone Regional Medical Center) is the closest thing on our roster to a true acting internship. You are managing patient care from admission to discharge, rounding daily, and presenting cases directly to the attending, inside an active IM residency environment capped at just 3 students. At $3,500, it is priced accordingly for the depth of hands-on responsibility it offers.

Two Psychiatry Options in Houston, at Very Different Price Points

Flourish Mental Health is a one-on-one rotation (max 1 student) with a child and adolescent psychiatrist who has particular expertise in autism-spectrum cases. It runs a full Monday–Friday, 9am–7pm schedule at $2,800.

The second Houston option, Psychiatry with 30 Years of Experience, is broader in scope: the preceptor sees patients of all ages, from children through adults, and has served on the medical staff at the University of Houston providing psychiatric evaluations for graduate students. At $2,000 for a max of 2 students, it is one of the more affordable psychiatry placements on our entire roster.

Family Medicine with a Mentor Who Was an IMG Himself

The Oakbend Medical Center rotation near Houston stands out for one specific reason: the supervising physician was an IMG himself, and understands the residency-match journey firsthand. You will see up to 20 patients per day on a flexible schedule (daily or 3 days per week), with involvement ranging from observation to active participation in procedures as your skills develop. At $1,675, it is the least expensive rotation in this guide.

What Every Texas Rotation Includes

  • A personalized letter of recommendation on hospital or clinic letterhead
  • A $500 deposit applied toward your total balance, due before your start date
  • Standard required documents: updated CV, AAMC standardized immunization records, and your medical school transcript or ID

How to Choose Between Them

If your goal is Internal Medicine or Hospital Medicine and you want a rotation that reads as a genuine sub-internship on your ERAS application, McAllen is the clear choice (just apply early, since it caps at 3 students). If you are targeting Psychiatry, decide between depth (Flourish's one-on-one, subspecialty-focused mentorship) and breadth (the 30-years-experience preceptor's all-ages caseload). And if Family Medicine is your track and budget matters, Richmond offers the highest patient volume per dollar on this list.
