General Surgery Rotations for IMGs: OR Exposure, LORs, and What Matters
General Surgery is a demanding specialty to apply into as an IMG, and program directors tend to weigh three things heavily when they read your rotation experience: how much real OR time you got, who wrote your letter, and whether the site itself carries any institutional credibility. Our five General Surgery rotations each lean into a different one of those factors. Here is how they compare.
General Surgery Rotations at a Glance
| Rotation | Location | Setting | Max Students | Price |
|---|---|---|---|---|
| Larkin Community Hospital | Miami, FL | Inpatient + Surgical Centers | 2 | $3,400 |
| Wright Center Faculty | Silver Spring, MD | Clinic + Lab Skills + OR | 2 | $2,500 |
| Kissimmee | Kissimmee, FL | Clinic + Surgical Procedures | 2 | $3,200 |
| Baptist Health Fort Smith | Fort Smith, AR | Inpatient + OR | 2 | $2,500 |
| Former Program Director | Englewood, NJ | Clinic + Outpatient Surgical Center | 2 | $1,996 |
If Accreditation Matters Most: Miami
The Larkin Community Hospital rotation in Miami is the only ACGME-accredited site on this list, with rounds also taking place at Hialeah Hospital. You round alongside Caribbean students enrolled in accredited programs, in a genuinely competitive clinical environment, with real OR observation built in. At $3,400 for 4 weeks, it is priced for that accreditation and the networking access it opens up.
If Hands-On Skills Matter Most: Maryland
Most General Surgery rotations are observation-heavy by necessity. The Wright Center Faculty rotation near Washington, DC is the exception: alongside OR observation and didactic sessions, you get dedicated lab time on basic suturing, knot tying, and in-office ultrasound technique, under a board-certified surgeon with over 20 years of experience. At $2,500, it is the rotation to pick if you want to leave with tangible procedural reps, not just observation hours.
If Steady Clinic Reps Matter Most: Kissimmee and Arkansas
Two rotations are built around a consistent, predictable clinical schedule. Kissimmee, Florida runs a standard Monday to Friday, 9am to 5pm week, with pre- and post-operative evaluations, wound care, and suturing practice at $3,200. Baptist Health Fort Smith in Arkansas spends more than half its time in the hospital itself, with case exposure to colonoscopy, mastectomy, and colorectal surgery, at $2,500. Both are designed for international students seeking immersive, active participation rather than a purely observational experience.
If Your LOR's Author Matters Most: New Jersey
The Englewood, New Jersey rotation is led by a former Program Director and current Hackensack residency faculty member with over 20 years of surgical experience. The structured week (clinic early on, student-led didactics midweek, physician-led discussions on Fridays, and procedures at an outpatient surgical center throughout) is deliberately built to give that mentor real visibility into your clinical reasoning before they write your letter. At $1,996, it is also the least expensive General Surgery rotation on our roster.
What Every General Surgery Rotation Includes
- A personalized letter of recommendation on hospital or clinic letterhead
- A $500 deposit applied toward your total balance, due before your start date
- Standard required documents: updated CV, AAMC standardized immunization records, and your medical school transcript or ID
How to Choose
If you can only afford to prioritize one factor, think about what your application is missing. If it lacks institutional credibility, Miami's ACGME accreditation is worth the premium. If it lacks procedural detail, Maryland's lab sessions fill that gap directly. If it lacks a senior physician's endorsement, New Jersey's former Program Director is hard to beat at the price. And if you simply need consistent, high-volume clinical hours, Kissimmee and Arkansas both deliver that reliably.
