Key facts
- ECFMG requires that medical schools be accredited by an agency recognized by the World Federation for Medical Education (WFME).
- The requirement was announced in 2010 and its start date was moved to 2024.
- It is applied by graduation year, and the result shows up as the graduation years in your school's ECFMG Sponsor Note.
- Your school's accreditation status is outside your control, but you can check it early.
ECFMG requires that international medical schools be accredited by an agency recognized by the World Federation for Medical Education (WFME) for their graduates to be eligible for ECFMG Certification. It was announced in 2010 to encourage accreditation worldwide and its effective date was later moved to 2024. For you, it shows up as one thing: whether your school's ECFMG Sponsor Note covers your graduation year.
The short version #
- Rule: the school must be appropriately accredited, meaning by an agency that WFME has recognized.
- Who it affects: applicants whose schools do not meet it for their graduation year.
- How you find out: look up the school in the World Directory and read the ECFMG Sponsor Note.
Why it exists #
ECFMG wanted a quality signal that applies to the whole school, not only to individual graduates' exam results. It chose accreditation by agencies that meet WFME's standards for the accrediting process itself. The goal, in ECFMG's words, is to stimulate accreditation efforts worldwide and improve the quality of medical education.
How the timing works #
The requirement was announced years before it took effect so that schools and agencies had time to prepare. The effective date was moved back to 2024 (from an earlier date) because of the disruption of 2020. Since then, ECFMG has applied it to graduates by graduation year.
The practical effects:
- Schools that already had accreditation from a WFME-recognized agency generally did not need to be re-accredited just for this, unless they were due for reaccreditation.
- A school may have been recognized for some graduation years and not others, depending on when its accreditation took effect.
- A school still in the process of getting accredited may, for a period, be ineligible for its graduates.
How to check your situation #
- Search for your school in the World Directory (wdoms.org).
- Open the Sponsor Notes tab and find the ECFMG Sponsor Note.
- Read the graduation years listed and compare to yours.
- Read ECFMG's current accreditation page and the Information Booklet for any statement about your country or school.
- Ask your school directly about the status and expected timeline of its accreditation.
If you are still a student, do this before your final year. Do not wait until after graduation to find out.
What is different for IMGs #
Everyone applying to ECFMG as an IMG is subject to it. US-accredited schools are under a different system and do not need this. If you graduated from a Canadian school on or after July 1, 2025, ECFMG states that you are considered an IMG for US GME entry, so check what applies.
If your school does not meet it #
This is hard news, and the options are limited and individual. Do not rely on a forum or an agency that claims to know a workaround. Read the ECFMG Information Booklet and contact ECFMG about your situation. Practical points:
- Accreditation is not something an individual graduate can fix.
- Your school might later gain accreditation, which could change eligibility for later graduation years, not necessarily earlier ones.
- Planning around other paths (for example, a different degree route) is a decision to make with official guidance, not with anecdotes.
Common mistakes #
- Assuming a government recognition or ministry listing equals WFME-recognized accreditation.
- Confusing the school being listed in the World Directory with being covered by the requirement.
- Relying on news from the 2010 to 2020 period. Dates and details have moved.
What comes next #
Confirm your school's status with the World Directory guide, then continue to credential verification. The certification overview shows the full set of requirements.
Frequently asked questions
What does the accreditation requirement actually require?
That your medical school is accredited by an accrediting agency recognized by the World Federation for Medical Education, as ECFMG applies it to your graduation year.
Does it apply to me?
It depends on your school and graduation year. The reliable check is the ECFMG Sponsor Note on your school's World Directory listing, which states the graduation years covered.
My school is accredited by a local body. Is that enough?
Only if that agency is recognized by WFME (or otherwise meets ECFMG's criteria). A national accreditation that is not WFME-recognized does not by itself satisfy the requirement.
Can I still apply if my school is still working on accreditation?
ECFMG has said that individuals may not be eligible if their school is still in the process of obtaining accreditation, so confirm your school's status and your graduation year directly.
