Setting Up Your MyIntealth Account Step by Step

How IMGs create a MyIntealth account, verify identity with a notarized form, and avoid the passport and name mistakes that cause delays.

Key facts

  • MyIntealth is the portal where IMGs apply for ECFMG Certification, upload documents, and request an ERAS token.
  • You verify your identity by completing an Intealth Identification Form (IIF) and having it notarized, usually online.
  • You need a current, unexpired passport, and your name, date of birth and gender must match it.
  • You get one MyIntealth ID only, and it cannot be changed.

Your MyIntealth account is the front door to everything ECFMG does for you: certification, document uploads, exam-related services and your ERAS token. You create it online at the MyIntealth Applicant Portal, then prove your identity with a notarized form. Doing this carefully the first time saves weeks later.

Before you start #

Gather these items first. ECFMG asks for:

  • your name exactly as it appears on a current, unexpired passport
  • your date of birth and address of residence
  • your medical school information
  • a full-color photograph of your face
  • a scanned, full-color copy of the photo page of your passport

File types and size limits for the photo and passport scan are set by ECFMG and have been described differently in different documents, so check the limits shown during the application itself.

Step by step #

  1. Open the MyIntealth Applicant Portal and enter your email address to receive a verification code.
  2. Use the email address ECFMG already has on record if you used ECFMG's former online services (OASIS). That links your existing account to MyIntealth instead of starting a new one.
  3. Answer the prior-application question honestly. If you have ever applied to ECFMG, even under another name, say yes and enter your USMLE ID if you know it.
  4. Submit your account request with your details, photo and passport scan.
  5. Wait for processing. After your request is processed, ECFMG creates a personalized Intealth Identification Form (IIF).
  6. Complete the IIF and have it notarized. This is normally done through NotaryCam, a live online session with a licensed US notary. You need the same identification you uploaded.
  7. Release the notarized form so ECFMG can complete identity verification.

You have six months from the date your personalized IIF is created to finish notarization, so do not leave it sitting.

Your IDs #

ID What it is
MyIntealth ID Issued when you establish your account. You get one, it cannot be changed, and you can find it by logging in. ECFMG will not give it out by phone.
USMLE ID Since the USMLE service transition in January 2026, FSMB issues the USMLE ID to IMGs when they register for a first Step exam. IDs previously issued by ECFMG remain valid.

Keep a record of both, and keep your login email secure.

What is different for IMGs #

US medical graduates do not use this process. For IMGs, identity verification is a gate: you generally cannot move forward on credentials, certification or an ERAS token until the account is established and identity is verified. Starting early is the simplest way to protect your timeline. The ERAS token guide explains why.

Common mistakes #

  • Name mismatch. The most common cause of delays. Your name should read the same on your passport, medical diploma references and exam registrations. If your diploma name differs, plan for extra documentation and read the ECFMG guidance before you submit.
  • Expired or soon-to-expire passport. Renew before you apply.
  • Creating a second account because you forgot the first.
  • Ignoring emails. ECFMG communicates through MyIntealth and email. Check spam folders.
  • Using an unofficial site. Only use the portal linked from ecfmg.org. ECFMG does not endorse paid services that offer to set up your account for you.

What comes next #

With your account verified, confirm that your school and graduation year are eligible. See the World Directory guide, then the credential verification guide. For the big picture, return to the ECFMG certification overview.

Frequently asked questions

What is the difference between a MyIntealth ID and a USMLE ID?

The MyIntealth ID identifies your ECFMG account. Since January 2026 FSMB issues the USMLE ID when an IMG registers for a first Step exam, and previously issued USMLE IDs remain valid. You can see your USMLE ID in the My Profile section of MyIntealth.

I applied to ECFMG years ago under another name. What do I do?

Answer yes when asked whether you previously applied, even under a different name, and enter your USMLE ID if you know it. This links your records instead of creating a duplicate.

Can I create more than one account?

No. You are assigned one MyIntealth ID and it cannot be changed, so duplicates cause problems. If you are unsure whether you have an account, check before creating one.

What if the online notary does not work for me?

ECFMG describes an alternate process involving in-person notarization. Check the account establishment page for the current instructions.
