Key facts
- In the 2026 Match, categorical General Surgery offered 1,807 positions and non-U.S. citizen IMGs filled 7.1 percent of them (NRMP).
- Preliminary (PGY-1 only) surgery offered 1,207 positions, only 55.0 percent filled in the algorithm, and non-U.S. citizen IMGs filled 18.1 percent of them.
- A preliminary surgery year is a one-year position that does not guarantee a categorical spot afterwards.
- Matched non-U.S. citizen IMGs who preferred General Surgery had a median Step 2 CK of 256 versus 247 for unmatched, in NRMP's 2026 report.
General Surgery is competitive for IMGs, and the preliminary-year route that many IMGs use is a different product from a categorical position. In NRMP's 2026 Main Residency Match, non-U.S. citizen IMGs filled 7.1 percent of the 1,807 categorical General Surgery positions. They filled 18.1 percent of the 1,207 preliminary (PGY-1 only) surgery positions, but 543 of those preliminary positions went unfilled.
What General Surgery is #
General Surgery is training in operative and perioperative care of conditions such as abdominal, breast, endocrine, trauma and emergency surgical disease, with long hours and heavy operative responsibility. Residents train in a multi-year program; many go on to fellowships. Fit and tolerance for workload matter as much as competitiveness.
The 2026 numbers #
| Position type | Positions offered (2026) | Filled in the algorithm | Share filled by non-U.S. citizen IMGs | Share filled by U.S. citizen IMGs |
|---|---|---|---|---|
| Surgery (Categorical) | 1,807 | 99.8% (3 unfilled) | 7.1% (128 matches) | 4.7% (85 matches) |
| Surgery-Preliminary (PGY-1 only) | 1,207 | 55.0% (543 unfilled) | 18.1% (218 matches) | 6.0% (72 matches) |
Categorical positions grew from 1,622 in 2022 to 1,807 in 2026. NRMP's Charting Outcomes report counts 2,760 applicants who preferred General Surgery for 1,807 positions (1.53 per position), and 124 of 433 non-U.S. citizen IMGs who preferred it matched. That is about 29 percent, calculated from the report's counts, compared with 41.5 percent for all specialties.
Categorical versus preliminary surgery #
NRMP defines a categorical position as one that provides the full training required for board certification, and a preliminary position as a one-year position that provides prerequisite training for advanced programs or other specialties. A preliminary surgery year gives you a surgical year and a US reference, but you remain in the next Match with no assured categorical slot. See categorical vs preliminary positions and preliminary and transitional years.
What programs commonly look for #
- Exams. In NRMP's 2026 Program Director Survey, 92 percent of programs (all specialties) required IMGs to have taken Step 1 and 84 percent required Step 2 CK for interview consideration. Among those requiring Step 2 CK from IMGs, 32 percent required a target score.
- Step 2 CK. Among consenting non-U.S. citizen IMGs who preferred General Surgery in 2026, the median was 256 for those who matched (102 applicants) and 247 for those who did not (230 applicants), per NRMP. The ranges overlap.
- Application volume. NRMP's survey reports that General Surgery is among the specialties receiving the highest mean number of applications. Only 59 of 647 programs responded, so interpret specialty results cautiously.
- Surgical evidence. Letters from surgeons who observed you, operating-room experience, research with a surgical department and evidence of technical aptitude and stamina.
- Reliability and teamwork. Ranking decisions across specialties lean on interpersonal skills (89 percent) and interview interactions (84 percent), per NRMP.
Visa and exam realities #
- Visas. Programs differ on sponsorship and visa type. Check each program and read J-1 vs H-1B.
- Screens. Exam attempts, years since graduation and gaps are common filters (program screening filters).
- ECFMG certification. Requirements must be completed by the rank order list certification deadline (March 3, 2027 for the next Match, per NRMP). See ECFMG certification.
- Signaling. General Surgery is on AAMC's 2027 list with 15 signals. See program signaling explained.
How IMGs commonly strengthen a General Surgery application #
- Get surgical USCE with a real letter (how to get US clinical experience).
- Add research with a surgical team (how programs view research).
- Rank many programs. Matched non-U.S. citizen IMGs who preferred General Surgery had a median of 3 contiguous ranks versus 1 for unmatched in 2026 (consenting applicants).
- Decide your preliminary-year policy in advance. Know how you will decide between a categorical and preliminary offer at ranking time, and what you will do if you end the year with no categorical spot.
- Maximize Step 2 CK.
Red flags #
- Multiple exam attempts, long gaps, or an old graduation year without a clear explanation.
- Applying for categorical positions with no operating-room or surgical letters.
- Taking a preliminary position without a plan for what happens next.
Backup planning #
A realistic plan runs a second specialty or a preliminary pathway in parallel without weakening the main application. See backup specialties and applying to more than one specialty. Internal Medicine is a common alternative; a failed categorical attempt can still leave you with options in reapplicant strategy.
Frequently asked questions
Is General Surgery competitive for IMGs?
Yes. NRMP's 2026 data shows non-U.S. citizen IMGs filled 7.1 percent of categorical positions, and 124 of the 433 non-U.S. citizen IMGs who preferred General Surgery matched to it.
Should I take a preliminary surgery position?
Only with open eyes. Preliminary positions are one-year, were far more available and more IMG-accessible in 2026, but they do not guarantee a categorical position afterwards.
Does General Surgery use program signaling?
Yes. AAMC's 2027 ERAS list shows General Surgery participating with 15 signals. Confirm the current table on AAMC's page.
How many programs should I apply to?
NRMP does not give a number, but its data shows matched IMGs ranked more programs in their preferred specialty than unmatched ones. Use your own profile and visa needs to decide.
