Common ECFMG Delays and How to Avoid Them

The most frequent causes of delays in ECFMG certification for IMGs, from credential verification to OET results and name mismatches, with practical prevention steps.

Key facts

  • Most delays come from items outside your control, such as your school, test seats and document handling, so the fix is starting early and checking status often.
  • Name and identity mismatches are among the most avoidable delays.
  • OET results must reach ECFMG, so a missing MyIntealth ID or an unmatched record can hold you up.
  • Keep a log of every submission, date and reference number.

Most ECFMG delays are not mistakes at ECFMG. They are waits on someone else: your medical school, a test center, a courier or a records office. The way to avoid them is to start the externally dependent steps first, send consistent documents, and check status regularly. This page lists the usual causes, what each looks like, and how to reduce the risk. For the order of the steps, see the certification timeline.

The usual delay points #

Delay What it looks like Prevention
Slow credential verification Status shows ECFMG waiting on your school Contact the school's office before ECFMG does, give them the correct contact, and see the EPIC guide
Wrong or outdated school contact Requests bounce or sit unanswered Confirm the registrar's current email or address and the person responsible
Name mismatch Different spelling, order or missing names across passport, diploma and exam records Use your passport name everywhere and read the MyIntealth guide
Translation problems Documents not in English are returned or rejected Use translations that meet ECFMG's stated requirements, and check them before sending
Incomplete documents Missing pages, unsigned items, unreadable scans Check each requirement against the Information Booklet
OET result not matched The result exists but does not link to your ECFMG record Provide the correct MyIntealth ID at booking, and confirm receipt. See the OET guide
OET retake A sub-test falls short and you wait for a new seat Book early and keep a buffer before your target Match
Exam rescheduling A test center or date is unavailable See Prometric scheduling
Pathway window timing You miss a seasonal application window Check the Pathways page for current dates
ERAS token request Token not yet issued because conditions are not met Read the ERAS token guide

Delays that come from rule changes #

  • Service transitions. Since January 2026, IMGs register for USMLE Steps through FSMB. A request made in the wrong system is not handled. Use the current instructions on usmle.org.
  • Yearly booklets. The Information Booklet is reissued yearly. A process you learned from last year's guide may be out of date.
  • Fee changes. Paying an old fee amount can delay processing. See ECFMG fees.

A prevention routine #

  1. Open a log. Record each submission, date, amount, reference and who you contacted.
  2. Verify identity first. Check your name and date of birth on passport, diploma, transcript and exam registration before any submission.
  3. Start verification early. Send school requests the moment your account allows it.
  4. Check status weekly. In MyIntealth, read messages as well as status.
  5. Escalate calmly. If something is stuck, email your school with a short specific request, then contact ECFMG through your account with your details.
  6. Keep a buffer. Plan your last external step well before the deadline of the Match you want.

When a delay threatens your Match #

If a requirement may not finish before the deadline for your Match, read the NRMP's and your target programs' stated cutoffs, then decide whether to apply this year or next. This is a strategy call, so discuss it with a mentor. The ERAS timeline and what to do if you do not match cover the consequences.

What is different for IMGs #

Only IMGs deal with ECFMG, and their delays are mostly cross-border: school offices in other time zones, translation, courier timing and test-center access. That is why a long lead time matters more for IMGs than for US graduates.

Frequently asked questions

What is the most common reason ECFMG certification is delayed?

Credential verification, because it depends on your medical school responding. Starting early and following up with the school is the best protection.

Can I speed up ECFMG processing?

Not by paying or pressing, as far as we know. You can avoid avoidable delays by submitting complete, consistent documents and checking MyIntealth status.

What should I do if my status has not changed for weeks?

Check your MyIntealth messages for requests, confirm your school received ECFMG's request, then contact ECFMG through your account with your reference details.

Do delays affect my Match eligibility?

They can. Programs and the NRMP expect specific requirements to be met by their deadlines, so delays that push past those dates can affect the cycle you can participate in.
