Key facts
- Driver's licence rules are set by each state, so confirm requirements on your state DMV website.
- A REAL ID licence for a noncitizen is issued only for the period of authorized stay, or one year if there is no definite end, under the REAL ID rules.
- Car insurance is required in nearly all states, and a lack of US driving or credit history can raise the quote.
- Many residents can delay buying a car until they know their commute and first paycheck date.
Driver's licensing in the US is run by each state, so the exact documents, tests and waiting times depend on where you train. This page covers the common pattern and what is special for visa holders, then helps you decide whether you need a car at all. For anything specific, your state's motor vehicle agency website is the authority.
Do you need a car? #
Not always. It depends on your city and hospital.
- Big cities with transit: many residents manage without a car, using transit, bikes, hospital shuttles or ride services.
- Suburban and rural programs: a car is often needed for commuting between sites, for clinics, and for groceries.
- Rotations at several hospitals: ask the program how many sites you will rotate through and how residents travel.
Opinion, not rule: delay buying a car until you know your commute, your insurance quotes and your first paycheck date. The first weeks are cash-tight (see arrival checklist).
Getting a driver's licence #
What states commonly ask for #
State rules vary, so confirm each of these on your DMV site:
- Identity and status documents: passport, visa, I-94 record, and the DS-2019 or I-797. Many states check your immigration documents electronically.
- Proof of state residency: a lease, utility bill or an official letter. Some states accept an employer letter.
- Social Security number or a statement of ineligibility. Because residents are work-authorized, you will usually be asked for your SSN. If your card has not arrived, ask what the DMV accepts. See SSN and ITIN.
- Vision test and a knowledge (written) test. Many states also require a road test, and some waive tests for people with a licence from certain countries or states. This varies, so do not assume.
- Fees and appointment. Some DMVs require booking, and waits can be long.
Using a foreign licence at first #
States set how long a foreign licence is valid for visitors and new residents, and some expect an international driving permit or an official translation. Because the period and the definition of "resident" vary, check the state DMV page before you drive on your home licence. Insurance companies may also have their own rules.
REAL ID and temporary licences #
REAL ID is a federal standard for identification. For noncitizens, the REAL ID Act says a temporary licence may be valid only during the person's authorized stay in the US, or for one year if there is no definite end to that stay. DHS rules say visa expiry dates are not the relevant measure. In practice:
- Your licence may expire earlier than a standard one, matching the end date on your I-94 or DS-2019 or approval notice.
- You may need to renew in person with updated documents, including when your status is extended.
- A licence marked as temporary is normal for visa holders.
REAL ID details, including what you need for air travel and federal buildings, are on the DHS website. States have their own implementation, and an enhanced or non-compliant licence is possible in some. Your passport also remains a valid ID for domestic flights.
Car insurance #
Almost every state requires some form of liability insurance, and the minimum limits differ by state. Points for new arrivals:
- No US driving record can raise quotes, because insurers see you as higher risk. Some will count driving history from your home country, and some will not. Ask when you get quotes.
- No credit history can also affect pricing in states that permit credit-based insurance scoring. Some states restrict it.
- Compare several quotes and ask which discounts you qualify for (for example, safe-driver or bundling with renters insurance).
- Keep proof of insurance in the car or on your phone.
Buying or leasing a car #
- Lenders look at credit. With no history you may be offered high rates, a large down payment or a co-signer request. Read banking and credit first, and compare offers from your bank or credit union before the dealer's financing.
- Total cost matters: insurance, fuel, parking, maintenance, registration and local taxes sit on top of the payment.
- A used car you can pay for outright may avoid financing problems, but have it inspected by an independent mechanic.
- Watch for paperwork traps: read the contract, ask for the total price including fees, and do not sign anything you do not understand. Be careful with "buy here, pay here" lots and add-ons.
- Registration and title are done through the state agency, and rules vary.
- Leaving the US after training: think about what happens to a loan or lease if you move for fellowship or change visa status, since a lease early-termination fee can be large.
Safety and the road #
- Rules on seat belts, phone use, alcohol and child seats are strict, and penalties vary by state.
- Fatigue after long call is a real risk. Programs have duty-hour limits and policies on post-call rest. See adjusting to US hospital culture. If you are too tired to drive after a shift, ask about a ride, a call room or another safe option.
- Keep your licence, registration and insurance proof with you.
