Key facts
- In the 2026 Match, PM&R offered 601 positions across PGY-1, PGY-2 and Physician (R) types, and non-U.S. citizen IMGs matched to 22 of them (NRMP).
- Every PGY-1, PGY-2 and Physician (R) PM&R position filled in the 2026 algorithm, which leaves nothing for SOAP.
- AAMC's 2027 signaling table lists PM&R with 20 signals.
- PM&R offers both PGY-1 positions that include the first year and advanced PGY-2 positions that need a separate first year.
Physical Medicine and Rehabilitation (PM&R, also called physiatry) is a small specialty in which IMGs fill few positions and nearly every position fills. In NRMP's 2026 Main Residency Match, PM&R offered 601 positions across all types and non-U.S. citizen IMGs matched to 22 of them.
What PM&R is #
Physiatrists diagnose and treat conditions that affect function, such as stroke, spinal cord and brain injury, musculoskeletal and sports problems, pain and neuromuscular disease, with an emphasis on function and quality of life. Work spans inpatient rehabilitation, outpatient clinics and procedures. Decide on fit before competitiveness (choosing a specialty).
How positions are listed in the Match #
NRMP lists PM&R under three position types (2026 Results and Data):
| Position type | Positions offered | Filled | Non-U.S. citizen IMG matches | U.S. citizen IMG matches |
|---|---|---|---|---|
| PGY-1 (includes first year) | 253 | 253 | 5 | 3 |
| PGY-2 (advanced) | 334 | 334 | 17 | 8 |
| Physician (R), PGY-2 reserved for prior GME | 14 | 14 | 0 | 2 |
Both PGY-1 and PGY-2 positions filled completely in 2026. Advanced positions start the year after the Match and need a separate first year, per NRMP's definitions (categorical vs preliminary). Check the position type on every program listing, since two programs with the same name can differ (preliminary and transitional years).
The 2026 numbers #
In NRMP's Charting Outcomes report for non-U.S. citizen IMGs, 828 applicants preferred PM&R for 601 positions (1.38 applicants per position). Of 39 non-U.S. citizen IMGs who preferred it, 21 matched to their preferred specialty, about 54 percent calculated from NRMP's counts, compared with 41.5 percent across all specialties. The number of IMGs is small, so a few applicants change the percentage a lot.
Among consenting non-U.S. citizen IMGs who preferred PM&R, 15 matched and 14 did not. Both groups had a median Step 2 CK score of 240, and matched applicants had a median of 5 contiguous ranks versus 1.5. With groups this small I draw no conclusion beyond what the table shows: the score did not separate the groups, and the number of ranks did.
NRMP's 2026 Program Director Survey had 16 responses from 135 PM&R programs (11.9 percent), and its narrative says PM&R directors most frequently endorsed (90 percent) often interviewing and ranking U.S. DO seniors. I could not extract PM&R-specific figures for IMGs, so I do not state any.
What programs commonly look for #
- Exams. Across all specialties in 2026, 92 percent of programs required IMGs to have taken Step 1 and 84 percent Step 2 CK for interview consideration (NRMP).
- PM&R exposure. A rehabilitation rotation or observership, and a letter from a physiatrist (turning USCE into a letter).
- Research or quality work in rehabilitation, neurology or musculoskeletal medicine (how programs view research).
- Interpersonal skills and teamwork, since PM&R works with therapists, nurses and social workers, and communication with patients and families is central.
Visa and screening realities #
- Sponsorship varies by program and visa type, and some programs do not sponsor. Ask before applying (J-1 vs H-1B).
- Screening filters (attempts, years since graduation) apply here too (program screening filters).
- Signaling. PM&R has 20 signals in AAMC's 2027 table, a larger number than in many specialties, so signals can be spread more widely (program signaling explained).
Strategy for IMGs #
- Get specialty exposure in the US if you can, and ask for a letter that describes your work with patients and teams.
- Apply to PGY-1 and advanced programs deliberately, with a plan and budget for the first year if you rank advanced programs.
- Interview widely. The contiguous-ranks difference is the clearest pattern in NRMP's PM&R report.
- Write a specific statement about rehabilitation, function and teamwork, not a generic interest in lifestyle.
- Plan a backup. Internal medicine, neurology or family medicine need separate statements and letters (applying to more than one specialty, backup specialties).
Red flags #
- Little evidence you understand what physiatrists do.
- Applying only to advanced programs with no first-year plan.
- Multiple exam attempts or long clinical gaps without explanation.
- Relying on SOAP: PM&R had no unfilled PGY-1 or PGY-2 positions in the 2026 algorithm.
Frequently asked questions
Is PM&R easier for IMGs than other specialties?
Not by NRMP's 2026 numbers. Non-U.S. citizen IMGs matched to 22 of 601 positions, and 1.38 applicants preferred PM&R per position, so it is a small and competitive field for IMGs.
Do I need a preliminary year for PM&R?
For advanced PGY-2 positions, yes. PM&R also has PGY-1 positions that include the first year, so check the position type on each program.
Can I switch into PM&R from another specialty?
Some physician-reserved PGY-2 positions exist (14 in 2026), but they are few and limited to people with prior graduate medical education.
Does PM&R use program signaling?
Yes. AAMC's 2027 list shows Physical Medicine and Rehabilitation with 20 signals. Confirm the current table on AAMC's page.
