Key facts
- In the 2026 Match, Radiology-Diagnostic offered 1,268 positions across PGY-1, PGY-2 and Physician (R) types, and non-U.S. citizen IMGs matched to 111 of them (NRMP).
- Most Diagnostic Radiology positions are PGY-2 advanced positions (1,083 of 1,268), so most applicants also need a separate first year.
- AAMC's 2027 signaling table lists Diagnostic Radiology with 6 gold and 9 silver signals, and Transitional Year with 12.
- In NRMP's 2026 report, 102 of 175 non-U.S. citizen IMGs who preferred Radiology-Diagnostic matched to it, though the consented subgroup is small.
Diagnostic Radiology is a small, competitive specialty in which IMGs fill a modest share of positions, and its structure adds a second task: finding a first clinical year. In NRMP's 2026 Main Residency Match, Radiology-Diagnostic offered 1,268 positions across all position types, and non-U.S. citizen IMGs matched to 111 of them.
What Radiology is #
Diagnostic radiologists interpret imaging (X-ray, CT, MRI, ultrasound, nuclear medicine and more) and advise the clinicians who order it. Patient contact is lower than in most clinical specialties and the work is screen-based. Interventional Radiology is a separate integrated pathway with its own NRMP listing and signal count (8 in AAMC's 2027 table), covered here only in passing. Decide on fit first (choosing a specialty).
How positions are listed in the Match #
NRMP lists Radiology-Diagnostic under three position types. The 2026 figures below come from the Results and Data report.
| Position type | Positions offered | Filled | Non-U.S. citizen IMG matches | U.S. citizen IMG matches |
|---|---|---|---|---|
| PGY-1 (includes the first year) | 156 | 148 | 19 | 8 |
| PGY-2 (advanced) | 1,083 | 1,066 | 91 | 30 |
| Physician (R), PGY-2 reserved for prior GME | 29 | 24 | 1 | 2 |
So about 85 percent of the positions are advanced positions that start the year after the Match, per NRMP's definition (categorical vs preliminary). An advanced applicant ranks the radiology program and also needs a PGY-1 position somewhere, which is normally a preliminary or transitional year (preliminary and transitional years). Some institutions link their advanced program to a preliminary program inside the Match, but NRMP's pages do not say every program does, so ask each one.
I did not find a separate early-match process for Diagnostic Radiology in the sources I opened, so I do not claim one exists. Check the current ERAS and NRMP calendars each year.
The 2026 numbers #
In NRMP's Charting Outcomes report for non-U.S. citizen IMGs, 1,419 applicants preferred Radiology-Diagnostic for 1,268 positions (1.12 applicants per position). Of the 175 non-U.S. citizen IMGs who preferred it, 102 matched to their preferred specialty, about 58 percent calculated from NRMP's counts, compared with 41.5 percent across all specialties. That figure counts a different group from the position-type table (it is based on the first-ranked specialty), so the two sets of numbers do not match exactly.
Among consenting non-U.S. citizen IMGs who preferred Radiology-Diagnostic, the median Step 2 CK score was 250 for those who matched (75 applicants) and 251 for those who did not (52 applicants). Matched applicants had a median of 5 contiguous ranks versus 1.5 for unmatched, and a median of 6 publications versus 4. These are medians for small groups, and NRMP warns that research counts are self-reported and unverified. They show that the Step 2 CK score alone did not separate the two groups, not what a program will require of you.
NRMP's 2026 Program Director Survey had only 31 responses from 228 Radiology-Diagnostic programs (13.6 percent), and I could not extract Radiology-specific IMG-openness figures from the narrative report, so I do not give them. Look at the survey's specialty tables on NRMP's page.
What programs commonly look for #
- Exams. Across all specialties in 2026, 92 percent of programs required IMGs to have taken Step 1 and 84 percent required Step 2 CK for interview consideration (NRMP). Of those requiring Step 2 CK, 32 percent required a target score for IMGs.
- Radiology exposure. Radiology electives, observerships or research in a radiology department, and a letter from a radiologist (observership vs externship).
- Research. Radiology programs are often academic, and imaging research is a common way to show interest (how programs view research).
- A credible first year. Choose the clinical year deliberately, since programs read how you performed in it.
Visa and screening realities #
- Sponsorship is decided program by program and varies by visa type. Ask before applying (J-1 vs H-1B).
- Screening filters such as exam attempts and years since graduation apply here as elsewhere (program screening filters).
- Signaling. AAMC's 2027 table gives Diagnostic Radiology 6 gold and 9 silver signals and Transitional Year 12. See program signaling explained and AAMC's page for how gold and silver work.
Strategy for IMGs #
- Apply to Radiology and a first year in parallel. Build a rank list that covers both, and do not assume a radiology match secures a first year, or the reverse.
- Interview broadly. Contiguous ranks separated matched from unmatched applicants in NRMP's report.
- Get radiology-specific output: a rotation or observership with a letter, and a small project.
- Plan the preliminary year for your goals. A transitional or medicine year builds relevant clinical credibility. Visa and contract dates for a one-year position matter too (visa timeline after the Match).
- Have a backup. A second specialty needs its own statement and letters (applying to more than one specialty, backup specialties).
Red flags #
- Applying without a plan for the clinical year.
- No radiology evidence, or a statement that treats the specialty as a lifestyle choice.
- Multiple exam attempts or long clinical gaps without explanation.
- Counting on SOAP: in 2026 only 8 PGY-1 and 17 PGY-2 radiology positions were unfilled in the algorithm.
Frequently asked questions
Is Radiology realistic for an IMG?
In NRMP's 2026 data non-U.S. citizen IMGs did match to Diagnostic Radiology, but they filled a small share of positions and the specialty is competitive, so treat it as possible but demanding and plan a backup.
Do I need a separate preliminary or transitional year?
For most positions, yes. The majority of Diagnostic Radiology positions are PGY-2 advanced positions, so you also need a clinical first year unless a program offers a position that includes it.
Is there a separate early match for Radiology?
I could not verify one. NRMP's 2026 report lists Radiology-Diagnostic inside the Main Residency Match, so check the current ERAS and NRMP calendars for any specialty-specific steps.
Does Radiology use program signaling?
Yes. AAMC's 2027 list shows Diagnostic Radiology with 6 gold and 9 silver signals. Confirm the current table on AAMC's page.
