Internal Medicine Sub-Internships for IMGs: A Complete Guide

A "sub-internship" (often shortened to "Sub-I," and also called an acting internship) is a clinical rotation where a student takes on intern-level responsibilities under supervision: admitting patients, writing daily notes, presenting on rounds, and following cases from admission through discharge. For an IMG, a strong Sub-I is one of the clearest ways to show a program director you are ready to function at a resident's pace on day one.

The True Sub-Internship: South Texas Health System

The McAllen, Texas rotation is the one on our roster that is literally titled a Sub-Internship, and it earns the name. You manage patient care from admission to discharge, actively participate in daily rounds and treatment planning, and present your cases directly to the attending physician, inside a hospital system with an active IM residency program. Capped at just 3 students, it is the most hands-on IM experience we offer, at $3,500.

Combined Inpatient and Outpatient: New York

The Newburgh, New York rotation at St. Luke's Cornwall Hospital's affiliated campus blends inpatient and outpatient IM over 4 weeks, with a supportive, resident-readiness-focused mentor and an IMG-friendly hospital that has its own residency program. It is also one of the few rotations on our entire roster with housing included: at $4,150 for rotation plus housing, capped at 2 students, it is priced as a complete package rather than tuition alone.

Rounds with the Chair of Medicine: Tennessee

The Winchester, Tennessee rotation pairs you directly with the Chair of Medicine at Highpoint Health St. Thomas Hospital for 100% inpatient rounds, plus time at an on-campus rehab center. You will work alongside US medical students during their own core rotations and take part in in-depth discussions of diagnostic reasoning and treatment planning. At $2,700 with room for 4 students, it is the most accessible price point of the three, and it comes with occasional hospital lunches and desserts as a small added perk.

What Every Internal Medicine Rotation Includes

  • A personalized letter of recommendation on hospital letterhead
  • A $500 deposit applied toward your total balance, due before your start date
  • Standard required documents: updated CV, AAMC standardized immunization records, and your medical school transcript or ID

How to Choose

If you want the closest possible simulation of intern-level responsibility for your ERAS application, South Texas is the strongest match for the "Sub-I" label. If you want a gentler ramp that still includes outpatient exposure and comes with housing solved for you, New York is the more complete package. And if you want direct, sustained access to a department chair and a slightly lower price point, Tennessee is worth a close look.
