Psychiatry Clinical Rotations for IMGs: What to Expect and How to Choose

Psychiatry has become one of the more competitive specialties for IMGs in recent Match cycles, which means a strong clinical rotation matters more than ever. Programs want to see that you understand outpatient psychiatric practice, can build rapport with patients, and can hold your own in case discussions. Here is every psychiatry rotation USMLE Wise currently offers, and how to think about choosing between them.

One-on-One Mentorship: Flourish and Ramapo Ridge

Two rotations on this list cap out at a single student, which means genuinely undivided attention from your preceptor. At Flourish Mental Health in Houston, you work full days (9am to 7pm, Monday through Friday) with a child and adolescent psychiatrist who has particular expertise in autism spectrum cases. At Ramapo Ridge Psychiatric Hospital in Wayne, NJ, your preceptor is the hospital's Department Head, with a subspecialty in geriatric psychiatry and substance use disorders. Both require a serious time commitment (Ramapo Ridge asks for a minimum of 30 hours per week), but you leave with a letter written by someone who watched you work every day.

Academic Affiliations: UNC and Wisconsin

If you want a letter that carries institutional weight, look at the two academically affiliated rotations. The UNC Chapel Hill rotation is led by a Medical School Associate Professor who spent over 20 years as Clinical Director of UNC's counseling and psychological services, and exposes you to emergency psychiatry, addiction medicine, precision medicine, and ecopsychology in a single placement. The Mequon, Wisconsin rotation is run by a Medical Director who previously served as Assistant Professor of Psychiatry at the University of Wisconsin-Madison School of Medicine and Public Health, and mixes telehealth sessions, in-person clinic, EMR training, and lectures on ADHD. Both are priced at $1,800, making them the most affordable psychiatry placements on our roster relative to the seniority of the mentor.

Broad, All-Ages Exposure: Houston

The second Houston rotation is led by a Medical Director with 30 years of clinical experience who has also served on the medical staff at the University of Houston, providing psychiatric evaluations and case management for graduate students. Rather than a subspecialty focus, this one gives you a diverse caseload spanning children, adolescents, and adults, which is useful if you have not yet settled on a specific psychiatric subspecialty.

What Every Psychiatry Rotation Includes

  • A personalized letter of recommendation on clinic letterhead
  • A $500 deposit applied toward your total balance, due before your start date
  • Standard required documents: updated CV, AAMC standardized immunization records, and your medical school transcript or ID

How to Choose

Start with the subspecialty you are most drawn to: child and adolescent care points to Flourish, geriatric psychiatry and substance use points to Ramapo Ridge, and emergency or addiction psychiatry points to UNC. If you want the strongest possible institutional name on your LOR, UNC or Wisconsin are your best options at the lowest price point. And if you are still deciding on a subspecialty, the broad, all-ages caseload in Houston gives you the widest exposure to figure it out.
