Family Medicine Rotations for IMGs: Building Your Primary Care Foundation
Family Medicine is one of the broadest specialties an IMG can train in, and also one of the more IMG-friendly ones to match into. Because the specialty itself spans everything from pediatrics to geriatrics to minor procedures, the rotations that prepare you for it vary just as widely. We currently offer six Family Medicine placements across five states, and no two look alike.

Breadth Across the Lifespan: New Jersey
The Englewood, NJ rotation, led by the hospital's Chief of Family Practice, is board-certified across Family Medicine, Geriatric Medicine, and Hospice and Palliative Medicine. You will spend time across an outpatient clinic, assisted living facilities, and nursing homes, a genuinely unusual mix of care settings for a single 4-week placement, at $1,996.
High Patient Volume: Texas
If reps matter most to you, the Richmond, TX rotation delivers up to 20 patients a day on a flexible schedule (daily or 3 days a week), under a mentor who was an IMG himself and understands your path firsthand. At $1,675, it is also the least expensive rotation on this entire list.
Academic Credibility: Virginia and New York
Two rotations pair you with physicians holding notable academic titles. In Alexandria, VA, your mentor is a Georgetown University School of Medicine Associate Professor with 39 years of active practice and five current clinical professorships. In New York City, you split time across two clinics with a board-certified physician who also holds the title of Visiting Attending Physician with Mount Sinai Hospital. Both letters carry institutional names that program directors recognize on sight.
Inpatient Exposure Within Family Medicine: Florida
Family Medicine does not usually include hospital time, which makes Florida's two rotations distinct. The Larkin Community Hospital rotation, run by the FM Program Director himself, combines inpatient rounding, outpatient clinic, telemedicine, and weekly online didactics, at $4,196 for up to 6 students. The Keralty Hospital rotation in Miami also mixes inpatient and outpatient time at $3,800, though it requires USMLE Step 1 to be completed first, and the LOR is performance-based rather than guaranteed.
What Every Family Medicine Rotation Includes
- A personalized letter of recommendation on hospital or clinic letterhead (conditional at Keralty)
- A $500 deposit applied toward your total balance, due before your start date
- Standard required documents: updated CV, AAMC standardized immunization records, and your medical school transcript or ID
How to Choose
If you want your application to show the widest possible primary care skill set, New Jersey's mix of clinic, assisted living, and nursing home care is hard to match. If your budget is tight and you want maximum patient volume, Texas wins on both counts. If you want an academically prestigious letter, choose between Virginia and New York based on which coast fits your logistics. And if Family Medicine is your track but you also want hospital-based experience on your record, the two Florida rotations are your only options that offer it.
